AWARDS BANQUET SOUVENIR BOOKLET ADVERTISEMENTS
For persons interested in placing an ad in Unity PAC's souvenir booklet to be
published in conjunction with the Awards Banquet, please note the following
terms and conditions. Ad sizes and prices for each size are listed below
Camera ready ad copy must be submitted or there will be a $25 charge for
layout of the ad copy Photos may be included in camera ready copies without additional charge. There will be a charge of $15 each for any photos included in non-camera
ready ad copy Deadlines for submission of ad copy and all fees is September
7, 2009 Deadline for approval of proof is September 7, 2009. Failure to indicate
approval of proof by the deadline, will be deemed acceptance of the proof as
accurate
You may pay for your ad by mail or online. If you do not pay online, you must make your check payable to Unity PAC and mail your ad order form and check as follows:

Please click on each "Pay Pal" button below that matches the type of ad for which you are paying. There will be a small handling or service charge added to your account to cover the cost of handling your online payment. The amount of the handling charge will appear on your account upon checkout and prior to your login to "pay pal" for final authorization of charge to your debit or credit card.
After you have made your payment, you will be directed to complete and submit an order form providing additional details about your order. You will also be directed to e-mail your ad copy and any photos that you want us to include in your ad.
Upon receipt of your order and payment, you will receive a confirmation by e-mail along with a proof of your ad copy, that must be returned within four days with your advice as to any changes.
